To the right, are forms for people that are wanting photographs taken of themselves. Please print all of the documents out, and fill out only what is necessary. The top document has the permission form to let me use and publish the photo's I take of you (basically post them online, and use them in advertising) -- if you are NOT wanting me to use them in that way, please leave that form blank, and print and fill out the third form. (Model Agreement-Not Use Pictures) Note: I know it may seem unnecessary to have both of these documents, considering they seem so much alike. But they are for two different things.
